First Visit – Important information
Getting to know you – Please arrive 30 minutes before your appointment to ensure that you have enough time to complete your medical history forms. Please bring with you:
- A list of all medications you are presently taking (i.e. heart, pain, or anticoagulant meds, etc.)
- Any dental X-rays you have
- Your referral slip
- Your insurance information (if you have insurance)
- Gov’t Issued ID such as Driver’s License
During your appointment, please share if you have a condition that may be of concern during surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or require medication prior to dental cleanings.
If your previous dentist took x-rays within the past 6-months, please request that they forward them to our office. If there is not enough time, please bring them yourself. If not, we will take them here.
Diagnosis and treatment options – We will thoroughly address any concerns or questions you may have. From there, we will proceed to provide you will a detailed diagnosis and different treatment options.
Starting treatment – Treatment most often can begin on the same day as your consultation. If not, we will schedule a date (or dates) together for administering the treatment. We will review treatment costs with you.
Important note for minors – A parent or guardian must accompany all patients under 18 at the consultation visit.
